Our Medical Services
Hometown may use Artificial Intelligence (AI) to assist in your dental, primary, or behavioral health care. Healthcare AI has many benefits in enhancing the delivery of care because it can, among other things, analyze medical data quickly and assist in diagnosis and treatment. If Hometown uses AI, it will be disclosed to you. You also may be proactive and let your caregiver know, AI should not be used. To learn more about healthcare AI click here.
For well and sick children, we provide services including growth and development assessment, safety, anticipatory guidance, immunizations, lead screening, anemia screening, nutrition and dental assessments. If indicated, we refer you to a pediatric specialist in your community to evaluate, advise and treat your child. These specialists include pediatric cardiologists, pediatric neurologists and ear, nose and throat specialists.
We may also refer your child for psychosocial, developmental, speech, hearing or other services provided through outside agencies. Children needing hospitalization are referred to Ellis Hospital and are cared for by the hospitalist with specialty consultants if necessary.
We provide well and sick visits to adolescents. Our services include health maintenance and assessments, immunizations, family planning counseling, and behavioral risk assessments. Specialists are available in your community to assist your adolescent’s primary care provider in giving care.
We provide acute sick visits to adults, plus periodic health evaluations including cardiovascular risk, diabetic assessments, cancer screening, substance abuse, HIV counseling, family planning, and immunization updates.
We emphasize preventative health services and following up on chronic medical problems. A full spectrum of specialists is available in your community through referral by your primary provider at the Center. If you need hospitalization, you will be referred to Ellis Hospital and be cared for by HHC providers with specialty consultants if necessary.
Since our elderly population may have a wide range of medical and socioeconomic problems, periodic health evaluations for the geriatric population are available at the Center. We offer preventative care, treat acute and chronic problems, and provide screenings including cardiovascular risk and diabetic assessments, substance abuse assessments, and vision and hearing assessments. Seniors also receive counseling for medication use and abuse and immunization updates.
HHC provides comprehensive family planning counseling, including patient education on all types of birth control measures. We can also give free information on safer sex practices and sexually transmitted diseases
(STDs), oral contraception, intrauterine device (IUD) insertion, diaphragm fitting, Depo‐Provera injection, and condom use. Condoms are available on-site. If you request a tubal ligation, you will be referred to our on‐site obstetrics consultant.
This department also maintains tracking logs on sexually transmitted diseases (STDs), abnormal pap smears, and mammograms. If you have an abnormal pap smear, you will be referred for a colposcopy procedure on-site by your GYN consultant. All our family providers offer primary care gynecology services, and complex cases are referred to our OB/GYN specialist.
We provide pediatric testing through a cooperative agreement with the Schenectady County Public Health Department to test at-risk children for elevated blood lead levels. All children are offered vision and hearing
screenings.
HHC provides personalized and comprehensive obstetric services dedicated to pregnancy, childbirth, and the postpartum period. Hometown physicians work closely in collaboration with our partners at Albany Med’s Maternal Fetal Medicine for high‐risk pregnancies and Bellevue hospitalists for labor and delivery at Bellevue Woman's Center. Before, during, and after pregnancy, care will be managed by our highly skilled obstetrics team while the new baby can be cared for by our pediatrics team.
Our case managers are members of your care team and work one-on-one with your provider and their support staff to provide health coaching and care coordination. The case managers assist our patients living with chronic health conditions to better understand and manage their well-being. They provide patient education and support to assist patients with reaching their care plan goals, better manage their medications, and assist with navigating the complex healthcare system.
