Our Behavioral Health Services
Since 2012, Hometown Health Centers has offered a comprehensive Behavioral Health program, providing counseling and support. Our staff offers behavioral health services to our Primary Care patients of all ages.
Hometown Health Centers (HHC) offers a comprehensive behavioral/mental health program providing on-site counseling and support services. The HHC team of specialists will counsel and treat children, adults, and senior citizens.
Hometown’s program is designed to be the bridge that will lead patients toward better health and emotional healing until a long-term plan can be coordinated. We work directly with HHC’s primary care providers and partners to achieve optimum outcomes.
Our Behavioral Health Services program is designed to be the bridge to help HHC patients receive the needed counseling and treatment quickly; until a long‐term plan can be coordinated. We communicate and collaborate with our Primary Care Physicians and care partners through our state‐of‐the‐art electronic medical record system.
The Hometown Behavioral/Mental Health Services Team offers counseling for many conditions, such as:
- Post-Traumatic Stress
- Substance Abuse
- School and Behavior Concerns
For more serious diagnoses, our staff in the Behavioral/Mental Health Services department can make referrals as necessary
