This privacy policy describes how the Hometown Health Centers website (hometownhealthcenters.org) collects and uses information from visitors. It covers what happens on the website itself.
It does not cover the privacy of your medical records as a patient. That is governed by HIPAA and is described in our Notice of Privacy Practices.
What we collect
Information you provide
If you fill out a form on this site (for example, a patient registration or consent form), the information stays in your browser. We do not receive or store anything you type into a printable form here. You bring the printed copy with you, fax it, or drop it off at a reception desk.
If you contact us by email or phone using the addresses or numbers on this site, we use that information to respond to your message and, when relevant, route you to the right person at Hometown Health Centers.
Automatic information
Our web server records standard request details (your IP address, browser type, the page requested, the referring page, and the time of the request) for security and reliability. These logs are kept for a limited time and are not used to build a profile of you.
Cookies
This site uses a small number of functional cookies to keep the site working — for example, to remember your language choice (English or Spanish). It does not use advertising or tracking cookies.
Embedded content
Location pages embed a Google Maps view of each clinic. When that map loads, Google may receive your IP address and basic browser details under their own privacy policy. No other third-party content is embedded on this site.
