Privacy Policy

How this website handles your information

This privacy policy describes how the Hometown Health Centers website (hometownhealthcenters.org) collects and uses information from visitors. It covers what happens on the website itself.

It does not cover the privacy of your medical records as a patient. That is governed by HIPAA and is described in our Notice of Privacy Practices.

What we collect

Information you provide

If you fill out a form on this site (for example, a patient registration or consent form), the information stays in your browser. We do not receive or store anything you type into a printable form here. You bring the printed copy with you, fax it, or drop it off at a reception desk.

If you contact us by email or phone using the addresses or numbers on this site, we use that information to respond to your message and, when relevant, route you to the right person at Hometown Health Centers.

Automatic information

Our web server records standard request details (your IP address, browser type, the page requested, the referring page, and the time of the request) for security and reliability. These logs are kept for a limited time and are not used to build a profile of you.

Cookies

This site uses a small number of functional cookies to keep the site working — for example, to remember your language choice (English or Spanish). It does not use advertising or tracking cookies.

Embedded content

Location pages embed a Google Maps view of each clinic. When that map loads, Google may receive your IP address and basic browser details under their own privacy policy. No other third-party content is embedded on this site.

How we use what we collect

  • To answer your questions and help you become or stay a patient
  • To keep the website secure and reliable
  • To meet legal and regulatory obligations that apply to a Federally Qualified Health Center

We do not sell your information. We do not share it with marketers or data brokers. We share it with a service provider only when that provider is helping us run the website (for example, our web hosting company), and only to the extent they need it to do that work.

Security

The website is served over HTTPS. Information you submit through forms on this site stays in your browser and is not transmitted to us until you choose to send it (by fax, in person, or by mail).

Children

This website is intended for a general audience. We do not knowingly collect personal information from children under 13 through the website. Pediatric patient information is collected at the clinic by parents or guardians and is governed by HIPAA, not by this policy.

Your choices

You can browse this website without giving us any personal information. Most browsers let you block cookies; if you block our functional cookies the language switcher and a few small features may not remember your previous selection.

Updates to this policy

If we change how the website handles information, we’ll update this page and adjust the date at the bottom. Material changes will also be flagged in a short note at the top of the page for at least 30 days.
