Hometown Health Centers believes every patient deserves to be treated with dignity. The Patient Bill of Rights describes the standards you can expect from us, and what you can do if those standards aren’t met.

Insurance & cost

We work with most insurance — and offer help when you don’t have any.

Accepted insurance

Hometown accepts a wide range of commercial, Medicaid, and Medicare plans. Review the current list of accepted insurances or call us with questions.

Enrollment help

Our on-site enrollment specialists can help you sign up for coverage through the New York State of Health Marketplace. Call us at (518) 370-1441, ext. 3348.

What to bring: photo ID, Social Security numbers (or immigration documents), proof of household income, proof of address, and any current insurance cards.

Sliding fee discount program

If you’re uninsured, underinsured, or low-income, you may qualify for our Sliding Fee Discount Program. Eligibility is based on household size and income; an application and proof of household income are required.

Privacy & records

Your records are yours — and they’re protected.

HIPAA & patient privacy

Hometown is committed to protecting the privacy of your health information in accordance with HIPAA. If you have questions or concerns about your privacy, you can reach out to us directly or report them to the U.S. Department of Health & Human Services.

Electronic medical records

We use the eClinicalWorks® EMR system, which lets your care team coordinate seamlessly across services. Through your patient portal, you can review your records, message your provider, and manage refills.

Free phone translation is available through our 800-number subscription, plus we have bilingual English/Spanish staff and English/Spanish patient literature.
