Two ways to book your appointment.

Hometown Health Centers patients are seen by appointment with their primary care provider. You can schedule online or call our Call Center. Same-day urgent visits are available when needed.

After hours coverage

We have on-call coverage 24 hours a day, 7 days a week. For an urgent medical concern outside regular hours, call our main number at (518) 370-1441 and follow the prompts.

Cancellations

To cancel an appointment, dial (518) 370-1441 and at the main menu press 2 for Medical, then press 3 to reach the appointments line. Phone menus are available in English and Spanish.

New patients

Welcome! Bring a photo ID, your insurance card (if you have one), and a list of any current medications to your first visit. Our front desk staff can help with paperwork in English or Spanish.

Your right to know

Good Faith Estimate.

You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost. Under the federal No Surprises Act, health care providers must give patients who don’t have certain types of health care coverage — or who are not using their insurance — an estimate of the bill for medical items and services.

Make sure your provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask any provider for one before you schedule. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute it.
