Hometown Health Centers helps people live healthier lives by improving health outcomes and health equity through quality primary care and preventative services. Anyone seeking care, regardless of income, will be helped and treated with respect by people who are mindful of cultural uniqueness and dedicated to continuous improvement.

Our Story

Half a century of community care.

1972

Practice granted operating certificate by the New York State Department of Health, opening as Carver Community Health Center.

1985

Carver issued a certificate of incorporation; new building opens at 305 Hulett Street.

2005

Hometown Health Centers opens at our current Schenectady Medical home — 1044 State Street.

2015

A 5,000 sq ft primary care building opens at 67 Division Street, Amsterdam — extending care into the Mohawk Valley.

2024

The new state-of-the-art Schenectady Dental facility opens at 1108 State Street — adding 30 chairs and full pediatric, surgical, and emergency care.

Today

For over half a century, Hometown Health Centers has been the primary care provider of choice for more than 20,000 local children and families across Schenectady, Amsterdam, and the Capital Region.
