Free Credit Monitoring and Identity Theft Protections for Two Years

As reported earlier this year – Change Healthcare, a Tennessee-based vendor once used by Hometown Health Centers, had a significant security breach. Hometown learned about like many healthcare organizations with reports in the media. This means someone may have accessed a limited amount of your information. Hometown, up until recently, used this vendor to send patients their invoices.
The company is in the final stages of completing its investigation.
According to the company, “Change Healthcare plans to send direct notice (written letters), at the conclusion of the data review, as required to affected individuals identified for whom Change Healthcare has a sufficient address. Please note, Change Healthcare may not have sufficient addresses for all affected individuals. The mailing process is expected to begin in late July after Change Healthcare completes quality assurance procedures.”
