HHC Joins AHA's Target: Type 2 Diabetes Outpatient Recognition Program
Thursday, March 3, 2022 — Hometown Health Centers (HHC) is pleased to announce that it recently signed on as a participant with the American Heart Association’s (AHA) Target: Type 2 Diabetes Outpatient Recognition Program. Through this program, HHC will focus on Type 2 diabetes control and cardiovascular risk factors within its patient population.
Type 2 diabetes is best described as an impairment in the way the body regulates and uses sugar as a fuel. This condition results in too much sugar circulating in the bloodstream, and eventually, these high blood sugar levels can lead to circulatory, nervous, and immune system disorders.
The collaboration with the AHA will help to ensure that Hometown is providing optimal care and meeting the latest industry standards for all patients with Type 2 diabetes.
