Hometown Health Centers Receives Two HRSA Awards
Wednesday, Sept. 29, 2021 -- Hometown Health Centers (HHC), the only not-for-profit, federally qualified health center in Schenectady County, has just been named a recipient of two Health Resources and Services Administration’s (HRSA) Community Health Quality Recognition (CHQR) awards.
The CHQR program recognizes health center program awardees and look-alikes that have made notable quality improvement achievements in the areas of access, quality, health equity, and health information technology (HIT) for the most recent uniform data system reporting period.
HHC received HRSA’s Health Center Quality Leaders award for achieving the best overall clinical quality measure performance among all health centers. HHC was in the top 21-30% of all health centers and is proud to be a bronze badge award recipient.
HHC also received HRSA’s Advancing HIT for Quality award. This award recognizes health centers that meet all criteria to optimize HIT services that advance telehealth, patient engagement, interoperability, and collection of social determinants of health to increase access to care and advance quality of care.
This is a great honor and huge testament to all the hard work that is done at Hometown Health Centers for the patients that we serve.
