Scheduling - appointments can be made by calling (518) 370-1441 or online through the Patient Portal HERE
As a Hometown Health Centers patient, you will be seen by appointment with your primary care provider by calling (518) 370-1441 during regularly scheduled health center hours, or you can make an appointment online through the Patient Portal. Patient appointments are scheduled by members of our registration staff or Call Center employees. You are encouraged to make a scheduled appointment with your provider, but in an effort to increase access to care in the community the Center does offer emergency urgent care and same-day walk-in appointments daily.
Patient scheduling policy.
Please be aware that Hometown Health has updated our patient scheduling policy. Important points for you to know:
- If you need to cancel an appointment, we ask for 24 hours notice. If you miss an appointment without contacting us to cancel you will be notified by phone or letter and counseled as to the consequences of missing another appointment.
- If you miss two appointments within six months, you can only schedule a call-in appointment for the same day.
- If you miss three or more appointments within six months, you will be seen by a provider, but must come to the health center for a “same-day” urgent care visit. You will be scheduled in this manner for one year.
- A missed medical/dental appointment presents a concern for both of us. For you, it causes a delay in treatment, and it prevents us from scheduling another patient who may need care. Thank you in advance for your cooperation.
After hours coverage.
Hometown Health Centers provides 24-hour, 7 days/week “on-call” provider coverage. If you need to reach a provider after hours, call the Hometown Health Centers main number (518) 370-1441, you will automatically be connected with the Ellis Medicine answering service.
